• Each patient prints his/her name, appointment time, physician's name that they are seeing on a numbered line. If the office is using the numbering system, patient takes number in the far left column next to their signature. The office staff removes the signed name label and adds it to the patient fiel or a workflow tracking system. Once the label is removed, the information is protected behind part one.
